How to Legally Change your Name in Massachusetts

If anyone has info for other states, please send it to me, and I will post it here, with proper attributing.

First off, the things you need:

1. a long form birth certificate, which means the copy with your parents' names on it. if you don't have this form of it, you can get it from your local city hall where you were born. in Springfield anyway, that means going into City Hall from the front, and it's the office on your left as you enter.
2. a money order for 180 dollars (as of October 2004 in Barnstable- thank you Jayson for updating the cost!), made out to the "Commonwealth of Massachusetts" - now as this cost may vary, please ask your Probate Divsion what the cost is for your county!
3. the paperwork itself, sent for free from your local friendly Probate and Family Court

Fill out the paperwork. Send it with the birth certificate and money order to your county's Probate and Family Court. Then the hardest part: waiting. It can take up to six weeks. Then, with the paperwork they send back in hand, go to Social Security first and then the RMV for new identification, and with that new identification, change everything else.

For Massachusetts residents, I have assembled the info I could find via the internet for each county's Probate and Family Court (or the nearest to that info I could find; anybody with more specific info please send it to me!). It's posted below so you can just easily pick up the phone and call for the paperwork you need. If any of the information has changed since I assembled it, please let me know so I can correct this page.
